As you know, I am currently in the process of applying for scholarships and colleges, and I would much appreciate if you would help me with this task. Having had me in your Anatomy and Physiology Class for two semesters, you know much of my academics and work ethic and it would be greatly beneficial to me if you would write me a letter of recommendation that I could use in my application processes.
As you are no doubt aware of the academic prowess I have made in your class over the past two semesters, I think you would be very capable of describing my work ethic and study habits, and how much progress I have made in this particular subject field from last semester to this one. Your recommendation would be able to accurately detail me as a worker and as a person.
